I understand that as a Prom Representative of Al's Formal Wear that I am required to hand out coupons for a prom discount to students at my school. I will receive $10 OFF my Prom Rental for each of my prom representative coupons that is redeemed. In order to receive a free rental for myself, at least twelve (12) of my coupons must be redeemed. I also understand that the coupon will not be valid without my signature, my prom registration number and the signature of the student redeeming my coupon. I also agree to discuss the Al's Formal Wear student services, as offered in the enclosed letter with the Senior Class Sponsor of my high school.

THE TUXEDO MUST BE RESERVED AT LEAST SEVEN (7) DAYS IN ADVANCE OF THE PROM DATE FOR THE COUPON TO BE VALID.

Any changes to this agreement or any coupon will make it null and void.
